The wound care case manager is responsible for assessment, intervention, discharge planning, resource utilization and management, program planning, data collection and analysis in order to meet the biopsychosocial needs of all wound care patients. In addition clinical responsibilities include but not limited to wound assessment and intervention per physician direction and education of the patient in their wound care management. Also, is responsible to perform hyperbaric oxygen therapy as needed.
Associates Degree. Bachelor of Science in Nursing , Respiratory or Physical Therapy preferred. Minimum experience of 3 years including a broad based clinical background with wound care or home care experience required.
Connecticut RN, PT or RRT license required. CWS or WOCN certification in wound care preferable. Documented experience in wound care and hyperbaric medicine. Certified Hyperbaric Technologist (CHT) within 1 year of completion of an Introduction to Hyperbaric Medicine Course.
