Home Care Setting:
$5,000 sign-on bonus for joining! Partial payment upon successful completion of the 90 day probationary period, balance paid upon completion of the first 6 months of employment.
1. Assessment of physical, emotional, socio-economic needs and environmental needs of patients and their families.
2. Identification of problems utilizing patient database, nursing process and physician's plan of treatment.
3. Develops a plan of care and interprets to the patient and family the implications of the diagnosis and the nature
of treatment consistent with the physician's plan of treatment.
4. Coordination of services with the patient, family and others involved in the care plan.
5. Regular evaluation of patient progress, prompt action when any change in the patient's condition is noted or
reported, and termination of care when goals for management are attained, or when termination of a case is
required according to Agency policy.
6. Observes, evaluates and reports to the physician the patient's physical and mental condition; reaction to drugs or treatments. Interprets to the physician social and physical factors in the environment, which affect physical care.
7. Identification of patient and family needs for other home health services and referral for same, when appropriate.
8. Responsibility for supervisor of the services rendered to the patient and family by the homemaker/home health
aide, according to State and Federal guidelines and Joint Commission standards, and submission of periodic
evaluation of such to the Home Health Aide Supervisor.
9. May visit and complete an assessment of assigned patients receiving homemaker/home health aide services as
often as necessary, based on the patient’s condition, but not less than every two (2) weeks for patients receiving more than homemaker/home health aide service, and not less frequent then every four (4) weeks for patients receiving homemaker/home health aide only.
10. Consults about individual or family health problems with other professional workers such as nutritionists,
respiratory therapists, social workers, and behavioral health nurse.
11. Identifies and interprets health needs of the community to the Agency.
12. Fulfills all compliance responsibilities related to the position.
This position requires a minimum formal education of RN Diploma and minimum of one year job-related experience
