Assists with the implementation of physical therapy services that identifies, prevents, remediates and rehabilitates acute or prolonged physical dysfunction or pain in the patient's home. Under the supervision of a physical therapist, delivers treatment including therapeutic exercise, physical modalities, patient and family education and procedures. Work is performed in the patient's place of residence.

Job Requirements

  • Associate Degree and minimum of zero to six months job-related experience.
  • CT State Physical Therapist Assistant license.  If visits to patient's homes are needed, a current valid driver’s license and access to a good working vehicle are required.
  • CPR certification.
  • Customer relation skills with patients and staff.  All PT modalities, use of a computer.
  • Ability to drive to patient homes within Fairfield County (includes but not limited to Norwalk, Danbury, New Milford, Bethel, Wilton)


Additional Job Requirements

  • Preferred FIM certification desired.
