Job Summary

Western Connecticut Medical Group is seeking a Geriatric Patient Care Coordinator 3 (APRN) for their WCMG Southbury Practice in Southbury, CT. It is a well-established practice and one of the largest geriatric practices in the state. You will work closely with 6 physician colleagues and 3 APRNs in a beautiful community with great shopping and easy access to I-84. It’s a unique practice because you will have the opportunity to take care of patients in the office, skilled nursing facilities, assisted living facilities and short term rehab facilities.

Job Summary:

The APRN provides medical care for patients in collaboration with a physician, assisting patients to recover from acute illnesses while minimizing the effects of chronic illnesses. Major accountabilities include, but are not limited to: 

  • Examines, evaluates, diagnoses and treats patients to provide expeditious, high quality, customer focused care.
  • Orders the appropriate laboratory and diagnostic procedures, and prescribes medical therapeutics as required to evaluate the state of the patient's medical condition.
  • Interprets and integrates data to determine the appropriateness of the therapeutic treatment plan.
  • Consults with supervising physicians in accordance with the practice's policies and medical standards to assure standards of medical care and patient satisfaction.
  • Records all symptoms, objective findings, assessment, treatment plan and other information that is appropriate in the patient's chart and signs the record.
  • Instructs the patient and family regarding the patient's diagnosis, testing, expected outcome and future prevention.
  • Evaluates the outcome of the treatment plan by reviewing diagnostic results with the patient through phone calls and return visits.
  • Performs total quality improvement processes and initiatives to achieve the Practice's mission.
  • Responsible for following departmental policies and procedures to assure service with proper risk management.
  • Fulfills all compliance responsibilities related to the position.
  • Performs other duties as assigned.

Job Requirements

  • This position requires a minimum formal education of Master Degree, State of CT RN and APRN license, CT Controlled Substances and Federal DEA Registration
  • Adult-Gerontology Nurse Practitioner preferred, previous geriatric experience preferred.
  • Willingness to work 8 hour shifts TBD between 7:30 am - 7 pm Monday through Friday.
