Job Summary

Western Connecticut Medical Group is seeking a Patient Care Coordinator 3 (APRN or PA) for their Danbury Primary Care practice in Danbury, CT. It is our largest Primary Care facility with close proximity to Danbury Hospital. It is a highly regarded, well-established internal medicine practice in a fast-paced, high volume location. You will work closely with 9+ physicians and a team of 5 Advanced Practitioners.

Job Summary:

The APRN or PA provides medical care for patients in collaboration with a physician, assisting patients to recover from acute illnesses while minimizing the effects of chronic illnesses. Major accountabilities include, but are not limited to: 

  • Examines, evaluates, diagnoses and treats patients to provide expeditious, high quality, customer focused care.
  • Orders the appropriate laboratory and diagnostic procedures, and prescribes medical therapeutics as required to evaluate the state of the patient's medical condition.
  • Interprets and integrates data to determine the appropriateness of the therapeutic treatment plan.
  • Consults with supervising physicians in accordance with the practice's policies and medical standards to assure standards of medical care and patient satisfaction.
  • Records all symptoms, objective findings, assessment, treatment plan and other information that is appropriate in the patient's chart and signs the record.
  • Instructs the patient and family regarding the patient's diagnosis, testing, expected outcome and future prevention.
  • Evaluates the outcome of the treatment plan by reviewing diagnostic results with the patient through phone calls and return visits.
  • Performs total quality improvement processes and initiatives to achieve the Practice's mission.
  • Responsible for following departmental policies and procedures to assure service with proper risk management.
  • Fulfills all compliance responsibilities related to the position.

Performs other duties as assigned.

Job Requirements

  • This position requires a minimum formal education of Master Degree, State of CT RN and APRN or PA license, CT Controlled Substances and Federal DEA Registration
  • Willingness to work 8 hour shifts TBD between 8 am - 7 pm Monday through Friday including flexibility to work one weekend per month (9am - 1pm Saturday).
