  • Provides home care services that identify, prevent, remediate and rehabilitate acute or prolonged physical dysfunction or pain.  
  • Evaluates and analyzes patients and therapeutic application, physical and chemical agents, exercise, instruction to patient/family and other procedures.

Full Time hours are 40 per week

Job Requirements

1.    Assists physician in assessing comprehensively the client's level of function.

2.    Performs physical therapy assessment including functional limitations, rooms, capabilities, mobility status,

       endurance status, ADL status.

3.    Identified client and family needs for other home health services and refers for same when needed.

4.    Provides appropriate treatment.  Treats clients to relieve pain, develop and restore function and regain optimum

       level of independence using appropriate equipment and modalities.

5.    Records on a visit basis, all evaluation data, treatments and client's response to therapy, provides client's discharge planning, when applicable.

6.    Participates in case management conferences with team members to coordinate physical therapy and total plan of care.

7.    Instructs client, family and other health care personnel in the client's treatment regiment, and documents to

       whom this instruction has been given.

8.    Provides or arranges for equipment required to increase the client's function and independence.

9.    Provides consultation and inservice education programs, when requested.

10.  Participates in quality improvement activities.

11.  Fulfills all compliance responsibilities related to the position.

Additional Job Requirements

  • Bachelor's Degree and minimum of one year job-related experience
  • Connecticut State Physical Therapy License
  • Valid drivers license
  • Desired (not required) criteria include:  Experience in qualified home health agency
  • Ability to travel to Patient's home within Fairfield County (Danbury, Bethel, Norwalk, or other areas may be included based on needs)
